The Saskatchewan Family Physician of the Year Award

If you know of an outstanding family physician please nominate them for the Saskatchewan Family Physician of the Year Award. We know that there are some outstanding family doctors in this province and each year we have an opportunity to recognize one of them.

Nomination Letter
For the Family Physician of the Year Award, each year, the Saskatchewan College of Family Physicians confers this award upon a member deemed to exemplify the ideals of family medicine. The award is adjudicated on the basis of the individual’s contribution through medicine generally, family medicine in particular, community through government, social, church or other activities as well as participation in the College of Family Physicians of Canada or the Saskatchewan College.

To qualify for nomination, a physician must have been in family practice for at least 15 years and a MEMBER of the College of Family Physicians of Canada for at least 10 years.

The recipient will be the Saskatchewan Chapter’s nominee for the 2025 Reg L. Perkin, Canadian Family Physician of the Year Award as one of the ten Family Physicians honoured by the CFPC.

Do you know someone who has done something particularly innovative or exceptional in the last 1 or more years? Saskatchewan has the opportunity to select up to 2 individuals to be recognized for their activities in the province or around the world with the Awards of Excellence. Please send nominations too scfp@sk-cfp.ca or complete the online application below.

Nomination Letter
The Awards of Excellence recognize CFPC members who in the past 12-24 months have made an outstanding contribution in a specific area pertaining to one or more of the following areas: patient care, community service; hospital or health care institutions; College activities (National or Chapter); teaching research or other elements of the academic discipline of family medicine.

Examples Include:

  • Charitable or humanitarian work
  • Outstanding teaching
  • Excellence in clinical practice
  • Innovative research program
  • Leadership during community outbreak of infectious disease
  • Establishment of training program in family violence
  • Innovations in developing a new program to address a health care need
  • Leadership role in a hospital or regional health care program

Recipients of the Awards of Excellence will be recognized by their Chapters at the Annual Scientific Assembly/Annual General Meeting and/or other Chapter events.

The recipients may also be recognized during Family Medicine Forum.

Nomination Process:

The Dr. Michael Krochak Award for Contributions to Family Medicine

This award is intended to recognize individuals (physician or non-physician) or groups, college members and non-members, that have made a significant contribution to family medicine in Saskatchewan through clinical research, education, or administrative activities. Please send nominations too scfp@sk-cfp.ca or complete the online application below.

SUBMIT AN ONLINE APPLICATION – The Dr Michael Krochak Award for Contributions to Family Medicine

Nomination Letter
This award is intended to recognize an individual, group or organization that has made a significant contribution to family medicine in Saskatchewan. This contribution may have occurred through clinical, research, educational, or administrative activities. The award is open to college members and non-members. It will recognize contribution made within the province or nationally.

Nominations will be accepted from Saskatchewan College of Family Physicians members. The recipient will then be selected by the Awards Committee.

The recipient will be recognized at the Chapter’s Annual Banquet held in conjunction with the Annual Scientific Assembly. The award will consist of complimentary banquet tickets for the individual and guest or designated recipient accepting the award.

Nomination Process:
Nominations are invited from CFPC members and the public and must be submitted to the Chapter office of the nominee. Nominations should include a brief introduction to the nominee, and why they are being nominated.

SCFP Resident Leadership Award

The SCFP Resident Leadership Award recognizes the leadership abilities of two (2) outstanding Residents from the Family Medicine and Enhanced Skills Programs. The award recipients will receive free registration for the 2025 Family Medicine Conference (FMC), being held in Regina on September 19, 2025, and up to $1,700 to attend the 2025 Family Medicine Forum (FMF), being held November 5– 8, 2025 in Winnipeg.  Pease submit your application to the following email scfp@sk-cfp.ca, by March 31st, 2025.Award Criteria

SCFP Medical Student Leadership Award

The SCFP Medical Student Leadership Award recognizes the leadership abilities of one outstanding medical student in their final year of study at the University of Saskatchewan.  The award recipients will receive free registration for the 2025 Family Medicine Conference (FMC), being held in Regina on September 19, 2025, and up to $1,700 to attend the 2025 Family Medicine Forum (FMF), being held November 5– 8, 2025 in Winnipeg.  Pease submit your application to the following email scfp@sk-cfp.ca, by March 31st, 2025.

SCFP Research Grant

This award was established to stimulate research and scholarship in Saskatchewan. This funding supports members in active practice to carry out research activities related to meeting the changing needs of their patients and communities within Saskatchewan. SCFP has a total fund of $20,000 available to support a maximum of two recipients annually.

National Awards Program

The College of Family Physicians of Canada has many different awards and honours available to celebrate the achievements and outstanding work of members by offering funding for scholarships and grants and by promoting their successes.

The Honours and Awards Program offers grants, scholarships and awards to recognize, and support family physicians, family medicine residents, and medical students committed to education, research, and excellence in the practice of family medicine.
